Case Information Form

After you have placed your order and submitted payment, please fill out this form with all the required information for your case.

We will need your first and last name, your date of birth, and a picture of you. From the spell’s target, we require the first and last name, date of birth, and also a picture. If you have ordered a break-up spell or another spell that involves additional people, please enter their information in the “Additional Persons” Field and also upload their pictures (if you have them). Alternatively, you can also email pictures to us via Email.
